As a therapist at CenterWell Home Health, you will help patients regain strength, mobility, and independence from the comfort of their homes. By delivering personalized care that focuses on functional improvement, you'll empower individuals to overcome limitations and enjoy a better quality of life.
As a Home Health Physical Therapist Assistant, you will work onsite in patients' homes to administer physical therapy treatment plans developed by the Physical Therapist. Your day involves delivering non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, and various modalities like heat, light, cold, water, and electrical treatments. You will be responsible for documenting client records and submitting clinical documentation to the office on time each week, while attending educational meetings and coordinating department in-services. A key part of your role is applying concepts of infection control and universal precautions in all patient care activities, and promptly notifying the Physical Therapist or Branch Director of any equipment failures, supply shortages, or staff and client changes.
