About the Role

As a Physical Therapist at CenterWell Home Health, you will join a caring community in Milwaukee, Wisconsin, serving patients across Washington and Ozaukee Counties. Reporting to the Clinical Manager, your mission is to help patients regain strength, mobility, and independence from the comfort of their own homes. You will deliver personalized care focused on functional improvement, empowering individuals to overcome limitations and enjoy a better quality of life.

This role offers a distinct approach to practice: you will enjoy 1:1 patient care with realistic caseloads you can sustain, backed by a team that supports your clinical decisions. We believe in fostering a lifestyle outside of work that you don't have to recover from. In this territory-based role, you will plan and administer prescribed physical therapy treatments, assess patient strengths, and develop rehabilitation programs consistent with physician goals. You will also collaborate closely with PTAs and Home Health Aides, providing clear care instructions and overseeing their performance.

Hiring Process & Application
