As a Home Health Physical Therapist Assistant, you will join a collaborative care team of nurses, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers. Reporting to the Clinical Manager, your role is to help patients regain strength, mobility, and independence from the comfort of their homes. By delivering personalized care that focuses on functional improvement, you will empower individuals to overcome limitations and enjoy a better quality of life.

You will administer physical therapy treatment plans developed by the Physical Therapist, working onsite in patients' homes. Your day will involve performing non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, and various modalities such as heat, light, cold, water, and electrical treatments. Beyond direct patient care, you will document client records, submit clinical documentation on time, attend educational meetings, and coordinate department in-services. You will also serve as a vital link by notifying the Physical Therapist or Branch Director of any equipment failures, supply shortages, or changes in staff or client status, all while applying strict concepts of infection control and universal precautions.

This role is part of Humana's driver safety program and requires a valid state driver's license, personal vehicle liability insurance, and reliable transportation. Additionally, as a patient-facing role, you will be required to participate in Humana's Tuberculosis (TB) screening program.
