As a Home Health Physical Therapist Assistant at CenterWell, you will become a vital part of our caring community. Reporting to the Clinical Manager, your mission is to help patients regain strength, mobility, and independence from the comfort of their own homes. By delivering personalized care that focuses on functional improvement, you will empower individuals to overcome limitations and enjoy a better quality of life.
In this role, you will administer physical therapy treatment plans developed by the Physical Therapist, utilizing non-complex active and passive manual therapeutic exercises, massage, traction, and various modalities. Your work involves documenting client records, attending educational meetings, and coordinating department in-services. You will apply concepts of infection control and universal precautions while collaborating with a team of healthcare professionals to support the geriatric population.
