About the Role

As a Home Health Physical Therapist Assistant, you will become a vital part of our caring community. Reporting to the Clinical Manager, your mission is to help patients regain strength, mobility, and independence from the comfort of their own homes. By delivering personalized care that focuses on functional improvement, you will empower individuals to overcome limitations and enjoy a better quality of life.

In this role, you will administer physical therapy treatment plans developed by the Physical Therapist, utilizing active and passive manual therapeutic exercises, massage, traction, and various modalities. You will be the face of our care team in the community, documenting client records, attending educational meetings, and ensuring the highest standards of infection control and safety. Your work directly supports our goal of providing accessible, comprehensive, and personalized whole-health solutions.

Hiring Process & Application

This role is patient-facing and requires adherence to specific safety protocols. Selected candidates will be required to participate in Humana's Tuberculosis (TB) screening program. Additionally, as this position involves driving to patient homes, you must be part of Humana's driver safety program, maintaining a valid state driver's license and personal vehicle liability insurance that meets state minimums or specific coverage limits.
