As a therapist at CenterWell Home Health, you will help patients regain strength, mobility, and independence from the comfort of their homes. By delivering personalized care that focuses on functional improvement, you'll empower individuals to overcome limitations and enjoy a better quality of life.
As a Home Health Physical Therapist Assistant, you will work onsite in patients' homes to administer physical therapy treatment plans developed by the Physical Therapist. Your day will involve delivering non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, and various modalities like heat, light, cold, water, and electrical treatments. You will play a vital role in documenting client records, submitting clinical documentation on time, and attending educational meetings to coordinate department in-services. Collaboration is key, as you will work alongside a team of healthcare professionals to notify the Physical Therapist or Branch Director of any equipment failures, supply issues, or changes in staff or client status, all while applying strict concepts of infection control and universal precautions.
CenterWell Home Health specializes in personalized, comprehensive home care for patients managing a chronic condition or recovering from injury, illness, surgery, or hospitalization.
