About the Role

As a Home Health Physical Therapist Assistant at CenterWell Home Health, you will become a vital part of our caring community. Reporting to the Clinical Manager, you will help patients regain strength, mobility, and independence from the comfort of their homes. By delivering personalized care that focuses on functional improvement, you will empower individuals to overcome limitations and enjoy a better quality of life.

In this role, you will administer physical therapy treatment plans developed by the Physical Therapist, working onsite in patients' homes. Your day-to-day involves performing non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, and various modalities such as heat, light, cold, water, and electrical treatments. You will also be responsible for accurate clinical documentation, attending educational meetings, and coordinating department in-services. Collaboration is key, as you will work closely with a team of healthcare professionals to ensure the highest standard of care while applying concepts of infection control and universal precautions.

Hiring Process & Application

This role is patient-facing and part of Humana's Tuberculosis (TB) screening program; selected candidates will be required to undergo TB screening. Additionally, as this position requires travel to patient homes, it is part of Humana's driver safety program. Candidates must possess a valid state driver's license and maintain personal vehicle liability insurance in accordance with state minimums or specific coverage limits.
