As a Home Health Physical Therapist, you will become a vital part of our caring community. Reporting to the Clinical Manager, you will help patients regain strength, mobility, and independence from the comfort of their homes. By delivering personalized care that focuses on functional improvement, you will empower individuals to overcome limitations and enjoy a better quality of life.
In this role, you will plan and administer prescribed physical therapy treatments onsite in patients' homes to attain the highest level of physical function. You will assess and document patients' physical strengths to support physician evaluations and guide treatment goals. You will develop and implement rehabilitation programs consistent with the physician's Plan of Treatment and the goals of the patient team. Additionally, you will provide Physical Therapist Assistants (PTAs) and Home Health Aides with clear written care instructions and oversee their performance and documentation. Your work will involve documenting patient observations, interventions, and evaluations promptly and thoroughly, while reporting progress to physicians, the Clinical Manager, the care team, patients, and families.
To be considered for this patient-facing role, candidates must be prepared to participate in Humana's Tuberculosis (TB) screening program. Furthermore, as this position is part of Humana's driver safety program, you must possess a valid state driver's license and maintain personal vehicle liability insurance in accordance with your residing state's minimum required limits (or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, whichever is higher).
