About the Role
The Home Health Care Consultant serves as the bridge between our home health program and the broader community. Under the direction of the Home Health Area Market Executive and Growth Director, this role is responsible for sourcing admissions by building and maintaining direct community contacts. You will act as an educator, informing healthcare providers and the general public about the benefits of our home health services.
This position is designed for someone who thrives in a dynamic environment, balancing strategic planning with hands-on community engagement. You will collaborate with clinical management staff to plan in-services and presentations, ensuring that referral sources and patients alike receive the support they need. The role requires a flexible schedule, including nights and weekends, and involves regular travel to manage territory effectively. Whether analyzing market data or presenting to potential partners, you will play a vital part in creating meaningful moments that honor the quality of life for our patients and their families.
