The Foster and Adoptive Department (FAD) is dedicated to empowering foster and adoptive families by providing advocacy, guidance, and essential resources. Our mission is to help families overcome challenges and achieve their goals through a strengths-based approach to care and service delivery.
As a Family Support Specialist, you will serve as a vital link between active foster and adoptive families and the community services they need. This position is designed to support families through a combination of home visits, assessments, and strategic referrals. You will manage a caseload of 25 to 30 cases, ensuring the safety and well-being of children while monitoring compliance with DFPS procedures and HHSC-CCR minimum standards.
Your day will involve coordinating STAR Health services, scheduling necessary assessments like CANS 3.0, and facilitating Treatment/Service Plan Meetings in collaboration with CPS and other team members. You will also provide personalized aftercare case management, connecting families with social work departments for community resources and educational support. Success in this role requires a clear-headed, decisive approach and the ability to maintain detailed documentation in our agency database on a weekly basis.
This is a home-based role with significant field assignments. You will be expected to travel throughout the Greater Houston Area to conduct home visits and support families in their communities. Reliable transportation is required for this position.
