This position serves as a vital support pillar for the Continuum of Care Team, dedicated to facilitating seamless discharge planning and ensuring efficient patient throughput. The role is designed to bridge the gap between hospitalization and post-acute care, working in close partnership with Case Managers, Transitional Care Coordinators, and Social Workers. The primary mission is to ensure that discharge plans are clearly communicated to patients and their families, identifying clinical barriers early to maintain continuity of care.
By connecting patients with necessary resources and fostering a positive experience throughout the discharge process, this role plays a critical part in the organization's commitment to patient well-being. The position operates within a fast-paced, multi-faceted team environment, requiring the ability to multitask, meet tight deadlines, and collaborate effectively with key stakeholders across Keck and Norris hospitals.
You will act as a central coordinator for patient transitions, ensuring a smooth journey from the hospital to home or the next level of care. Your day involves partnering with the Care Coordination team to manage communication at the point of discharge and conducting timely post-acute follow-ups to reinforce instructions. Under the supervision of the Transitional Care Coordinator, you will utilize various referral platforms to review post-acute referrals, analyze discharge summaries, and follow up on all assigned tasks.
A significant portion of your time will be spent engaging directly with patients via phone calls, utilizing established scripts and algorithms to assess their status and needs. You will also maintain frequent communication with clinic physician staff and post-acute providers to address specific patient requirements. Beyond clinical coordination, you will handle non-clinical aspects of discharge planning, such as arranging transportation, securing durable medical equipment, and identifying psychosocial barriers like housing needs.
The role requires active participation in departmental meetings, daily huddles, and triad huddles to align on priorities and assignments. You will be responsible for documenting all activities in the electronic Medical Record and assisting with patient transfers between levels of care. Flexibility is key, as the position involves floating between assignments and hospitals, with expectations for on-call coverage, weekend shifts, and rotations to manage fluctuating discharge needs.
