COMMUNITY SUPPORT WORKER

About the Role

This position is within the Clinical Social Work department, focusing on Behavioral and Mental Health. The role is dedicated to coordinating and providing essential services and resources to individuals and families to promote rehabilitation, recovery, and resiliency.

You will engage in patient care that encompasses supportive counseling, safety planning, and skill-building interventions. The position serves a diverse patient population, including children at risk of or experiencing serious emotional, neurobiological, and behavioral disorders, as well as adults with severe mental illness, chronic substance abuse, and co-occurring disorders. Assignments may span neonate, pediatric, adolescent, adult, and geriatric age groups.

Day-to-Day Impact

Your work will center on identifying strengths to aid recovery while addressing barriers to independent functioning. You will support individuals and families during crisis situations, providing personalized interventions to enhance their ability to make informed, independent decisions. Key aspects of the role include:

• Advocacy and Navigation: Eliminating barriers to access culturally competent medical services and assisting patients in securing community resources and financial assistance.
• Collaboration: Working closely with medical providers, multidisciplinary teams, and community resource providers to meet treatment goals.
• Life Skills Development: Facilitating skills in activities of daily living, interpersonal coping, socialization, and community functioning, including adaptation to home, school, work, and recreational environments.
• Wellness and Recovery: Serving as a mentor to instill hope, teaching symptom monitoring and management, and assisting individuals with medication knowledge and self-motivational skills.
• Service Planning: Assisting in the development of comprehensive service plans, including recovery management, crisis management, and advanced directives.
• Crisis Intervention: Assessing and intervening in crisis situations, facilitating the use of natural supports, and identifying alternatives to emergency department visits.

The role requires adherence to hospital and departmental policies, with a strong emphasis on patient safety, proactive improvement of safety protocols, and encouraging patients to actively participate in their own care.

Compensation Note

Compensation for this role is based on a number of factors, including but not limited to experience, education, and other business and organizational considerations. Shift differentials apply: 17% for weekday nights, 26% for weekend nights, and 15% for weekend days.
