This position serves as a vital bridge between clinical care and community living, dedicated to coordinating services that promote rehabilitation, recovery, and resiliency for individuals and families. The role focuses on identifying personal strengths to aid in the recovery process while addressing barriers that may impede independent functioning. You will support clients through crisis situations, providing personalized interventions that empower them to make informed, independent decisions.
The patient care assignments are diverse, potentially including neonates, children, adolescents, adults, and geriatric populations. You will work with individuals facing serious emotional, neurobiological, or behavioral disorders, severe mental illness, chronic substance abuse, and co-occurring disorders. The core mission is to facilitate the development of life skills, including daily living activities, socialization, and community functioning, while ensuring adherence to hospital and departmental policies.
Part of the Behavioral and Mental Health department, this role operates within a Patient Centered Medical Home (PCMH) or Patient Centered Specialty Practice framework. The team collaborates closely with medical providers, multidisciplinary members, and community resource providers to eliminate barriers to culturally competent medical services. The environment requires a proactive approach to patient safety, encouraging active patient participation in their own care and seeking opportunities to improve safety protocols.
Candidates must possess a Bachelor's Degree in a related discipline and at least one year of directly related experience. Essential credentials include a valid New Mexico Driver's License and CPR for Healthcare/BLS certification (to be obtained within 30 days). A UNM Vehicle Operator's Permit is required within 60 days. Bilingual skills in English, Spanish, Keres, Tewa, Tiwa, Towa, Zuni, or Navajo are considered nonessential but highly valued.
