About the Role
Premier Home Care Residential Service Agency is seeking a motivated and organized Community Outreach Coordinator to drive our field-based engagement efforts across Maryland. This field-based contract role is designed to support our mission by fostering vital connections with healthcare facilities, community partners, and referral sources.
As the face of our agency in the field, you will conduct in-person visits, manage communication with external stakeholders, and ensure accurate tracking of all outreach activities. This position is ideal for a dependable, detail-oriented professional who thrives working independently while managing a dynamic schedule of travel, follow-ups, and documentation. You will play a key part in building and maintaining the professional relationships that allow us to serve our community effectively.

Key Responsibilities

Requirements
