The Care Transition Liaison serves as the vital link between Tufts Medicine Care at Home and the broader community, including hospitals, nursing homes, physicians, patients, and families. Based on-site at Melrose Wakefield Hospital, this role is dedicated to generating home health and hospice referrals while acting as a strategic partner to referral sources.
In this position, you will thoroughly identify the needs of your designated territory, demonstrating a clear understanding of how decision-makers in the healthcare ecosystem operate. You will collaborate in a cross-functional environment to improve home health care services, performing pre-intake screens and facilitating patient assessments. Your day-to-day involves building intimate relationships with case management departments, presenting educational programs to hospital personnel, and ensuring seamless patient flow from admission to care delivery. You will act as a preferred partner rather than just a vendor, seeking opportunities to join clinical teams and committees where your expertise in post-acute care can benefit the account.
