The Client Care Coordinator is a shelter or temporary lodging site-based position serving as the bridge for individuals and families residing in these facilities. You will work with clients who have the highest acuities and greatest vulnerabilities, managing a housing caseload responsible for coordinating care to help end their current episode of homelessness.
In this role, you will coordinate care and support services for individuals and families presenting with complex substance use, mental health, housing, and medical care needs. You will ensure connections to existing Patient-Centered Medical Homes (PCMH) or Primary Care Providers (PCP), or facilitate connections to new services. Working as part of a multi-disciplinary team—including Patient Care Techs, Registered Nurses, Counselors, and Clinical Leads—you will serve as a nexus, coordinating care with community providers and stakeholders.
You will utilize housing search software systems to help individuals identify units once matched to housing. Your work will focus on addressing social determinants of health through housing and community integration programs, such as crisis care, residential treatment, and outpatient behavioral health services. You will document and track required interactions according to Arizona Administrative Code, ADHS/DBHS Provider Manual, CARF standards, and CBI policies.
