As a Social Services Coordinator, you will serve as a primary point of contact for individuals and families navigating complex social systems. This role exists to bridge the gap between clients and the resources they need, ensuring that barriers to care are addressed and goals are met. You will be the face of our organization in underserved neighborhoods, engaging with diverse populations from different cultural and linguistic backgrounds.
Your day will involve conducting intake assessments, developing service plans, and coordinating referrals to medical, behavioral health, education, and housing partners. Beyond direct client support, you will build strong referral networks with healthcare providers and community agencies to create integrated care pathways. You will also play a key role in identifying emerging community needs, sharing insights with leadership to inform new initiatives that drive continuous improvement.
We are looking for candidates who demonstrate cultural humility, strong communication skills, and the ability to handle sensitive, confidential information with professionalism and empathy. The interview process will focus on your experience in community-based settings, your approach to trauma-informed care, and your ability to navigate complex social service systems.
