The Case Manager II is a vital member of the primary care interdisciplinary team at LifeLong, dedicated to serving patients with complex health and social service needs. This role goes beyond traditional clinic settings, extending care into homes, hospitals, supportive housing, encampments, and shelters. As the lead implementer of Enhanced Care Management (ECM), you will conduct outreach, engage patients, and assess psychosocial needs to develop patient-centered care plans.
Your day involves building effective relationships with individuals facing multiple chronic conditions, such as HIV/AIDS, severe diabetes, or homelessness. You will empower patients through health education on harm reduction and disease mitigation, assist them in accessing public benefits and affordable housing, and advocate for their needs within the community. This position requires a flexible approach, often working outside of a traditional health center to meet clients where they are.
