This position is central to fostering a purposeful and engaging culture within our community. You will provide dementia care that includes adapted activities of daily living (ADLs), meaningful engagement, and communication to residents, all while following a person-centered individual service plan. Your role is to treat residents with respect and dignity, recognizing individual needs and encouraging independence.
In your day-to-day, you will partner with residents to assist with bathing, dressing, grooming, toileting, and transferring, while encouraging them to do as much of their own care as possible. You will serve meals in the dining room, potentially assisting in preparing dementia-friendly menus, and use prompting skills to encourage proper diet and hydration while respecting the resident's right to free choice. A key part of your mission is to know each resident's "Life Story," preferences, and historical routines to initiate and lead engagement programs, such as the Solace program, fostering socialization and friendships among residents.
You will also maintain accurate daily records regarding medication distribution, engagement interests, incidents, and observations. You will monitor the health and emotional condition of each resident, promptly reporting any changes to the supervisor. In emergency situations, you will follow proper procedures and respond positively to resident requests, including emergency pull cords and calls from family. Your work ensures a clean, safe, and orderly environment for all residents.

High school diploma or general education degree (GED) and three to six months related experience preferred and/or training in direct service with older adults living with dementia, or equivalent combination of education and experience. Knowledge of dementia, particularly of the Alzheimer's type.
