The Care Transition Specialist plays a critical role in ensuring the seamless initiation of services for newly admitted clients. As the first caregiver assigned to a new client, you will provide immediate coverage during the transition period prior to the assignment of a permanent caregiver. This position is essential for supporting timely admissions, maintaining continuity of care, and delivering a high-quality client experience.\n You will establish rapport with clients and families, implement the initial care plan, and observe and report client needs to the clinical and operations teams. Serving as a key liaison during the early stage of service delivery, you will help identify necessary adjustments to promote long-term success and effective caregiver matching. Your work directly supports agency growth, client retention, and service reliability by preventing delays in care initiation and reducing service gaps.
A core part of your day involves facilitating a structured and professional handoff to the permanent caregiver, ensuring clear communication and a smooth transition of care. You will operate under the direction of the Clinical Director and the VP of Operations, acting as a stabilizing force while permanent caregivers complete their onboarding and orientation requirements.
