The Child Welfare Case Manager is a pivotal role designed to provide full wrap-around case management services to LSF clients and families. This position is about more than just managing cases; it is about establishing genuine relationships with families while living out the agency's core values. The Case Manager serves as the integrator of all services and supports identified for each child, including therapy, mental health services, health and dentistry, developmental services, educational support, and permanency planning. The role requires a deep understanding of case ownership and the responsibility to facilitate trauma-sensitive transitions when necessary.
