About the Role
As the Director of Case Management, you will serve as a key member of our leadership team, combining fulfilling career opportunities with the chance to make a meaningful difference in the well-being of your local community. This role is designed for an experienced and compassionate healthcare professional seeking to align their expertise with personal values.
In this position, you will lead a team dedicated to care, compassion, and leadership. You will oversee the day-to-day operations of the Case Management department, ensuring the effectiveness of interdisciplinary plans of care and discharge planning. Your focus will be on census management, patient care outcomes, and key care indicators, while acting as a dedicated advocate for patients and their families. You will build strategic relationships with insurance companies, self-insured employers, and healthcare networks, while providing guidance and training to Case Managers to ensure high-quality service consistent with organizational standards.

Key Responsibilities
- check_circleAssume responsibility for day-to-day operations and human resource management of the Case Management department
- check_circleOversee the interdisciplinary plan of care and discharge planning process to ensure service effectiveness
- check_circleAct as a patient and family advocate to ensure services meet patient and family needs
- check_circleProvide guidance and support to Case Managers including training on caseload management and regulations

Requirements
