Mentor Community Services, part of the Sevita family, is a leading provider of home and community-based specialized health care. For over 50 years, our mission has been to help individuals with intellectual and developmental disabilities live full, independent lives. With 40,000 team members serving 50,000 individuals across the U.S., we are dedicated to providing quality services and individualized supports that foster growth and independence, regardless of physical, intellectual, or behavioral challenges.
You will join a mission-driven team where every person is valued and deserves a fulfilling career. As a Direct Support Professional, you will create meaningful relationships that motivate us all every day. Your work will take place within a single program or unit, typically supporting 4 to 6 individuals in residential settings or up to 10 in periodic programs.
In this role, you will be the backbone of daily life for the individuals you serve. Your day will involve delivering personalized support according to individual service plans, assisting with socialization, health maintenance, and skill development. You will help individuals navigate their daily activities, from personal care and housekeeping to recreational activities, transportation, and community integration. Whether accompanying someone to a medical appointment, preparing meals according to specific dietary needs, or simply sharing a moment of connection, your presence helps build a life well lived.
To join our team, candidates must meet specific state and company requirements, including being at least 18 years old and possessing a valid driver's license in good standing. All new hires are required to undergo drug testing prior to employment, as well as criminal background checks, including checks against the Central Registry of Offenders Against Individuals with Developmental Disabilities and the Child Abuse Registry Information (CARI). Successful candidates must also complete all state-mandated training and certifications within the required timeframes.
