About the Role

Based at the UCHealth Medical Center of the Rockies in Loveland, Colorado, this full-time Care Management position serves as a vital link within the MCR Care Management department. The role is designed to work collaboratively with physicians, interdisciplinary teams, individual clients, and families to promote positive health outcomes.

In this onsite capacity, you will perform a comprehensive care continuum process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to support an individual's health needs. Utilizing skilled communication, education, and resources, you will help clients examine patterns of health care needs, decisions, and lifestyle choices. The position provides focused support across various areas, including utilization management, the emergency department, acute care, ambulatory care, and specialty care.
