Based at UCHealth Memorial Hospital Central in Colorado Springs, this Care Management position within the MHS Care Management department serves as a vital bridge between clinical care and patient outcomes. Working onsite as a full-time Registered Nurse, you will collaborate closely with physicians, interdisciplinary teams, and individual clients to navigate the continuum of care. Your role involves assessing, planning, implementing, and evaluating health services to ensure quality, cost-effective outcomes for patients across utilization management, emergency departments, acute care, ambulatory settings, and specialty care.
In this fast-paced environment, you will utilize critical thinking and evidence-based practices to advocate for clients and their families. You will coach individuals on treatment options, community resources, and self-care skills, helping them develop independence appropriate to their age and developmental level. By facilitating communication among the healthcare delivery team, you will minimize service fragmentation and ensure clients are active participants in their own decision-making processes.
