Inpatient Care Managers are Registered Nurses who serve as the critical link between acute hospital care and the next level of treatment. This role exists to independently assure that patients are admitted to and transitioned to the correct level of care, ensuring accurate billing, reimbursement, and, most importantly, the delivery of quality, cost-effective clinical coordination.
As expert consultants and educators, Inpatient Care Managers support physicians and the broader healthcare team by managing complex transition planning needs. You will assess patient needs, develop comprehensive plans of care, and coordinate internal and community resources to optimize the utilization of healthcare resources while maintaining the highest standards of patient safety and care quality.
Your primary accountability involves anticipating and communicating safe transition plans for patients with complex care needs. You will independently conduct thorough, age-appropriate assessments that reflect psychosocial support systems, care needs, and health plan benefits. A significant portion of your day is dedicated to facilitating resolution of barriers to safe transfers through patient and team care conferences, ensuring that patients and families receive the right care at the right time.
You will act as a constant patient advocate, collaborating with interdisciplinary staff to ensure regulatory and compliance standards are met. This includes maintaining systematic contact with continuing care services to guarantee safe transitions across care settings. Whether working in Med/Surg, ICU, Emergency, or Utilization Management, you will utilize your clinical judgment to navigate high-pressure situations and provide culturally competent care.
Candidates must possess credentials from the primary work state at the time of hire. Additional credentials for secondary work states will be required post-hire. The role operates on a Sun-Sat schedule, including holidays and weekends.

Knowledge of appropriate utilization of acute hospital and Kaiser Permanente internal resources. Knowledge of Medicare and Medicaid regulations related to eligibility requirements for hospital, nursing facilities, home health, hospice, and DME. Knowledge of utilization management principles and tools. Demonstrated clinical judgment and customer-focused service skills. Knowledge of principles of patient teaching, disease prevention measures, and physical assessment as it relates to the needs of patient and the next level of care. Certified in Case Management. BSN or bachelor's degree and MSN.
