Key Responsibilities
- check_circlePerform in-home care management visits to execute care management plans
- check_circleMonitor biometric data and follow approved protocols for necessary interventions
- check_circleEncourage medication and treatment adherence through frequent patient contact
- check_circleEngage family and social support groups in patient education and care
- check_circleServe as the primary point of contact for patient questions during business hours
- check_circleProvide education and coaching on medications, conditions, diet, exercise, and lifestyle
- check_circleObtain vital signs during visits and escalate concerns to providers
- check_circlePerform post-op and hospital discharge visits to support vulnerable transitions
- check_circleReview and document patient updates and progress in the care management platform
- check_circleCoordinate with dialysis providers to ensure seamless transitions of care
- check_circleFacilitate conversations regarding proactive care decisions and Advance Care Plans
- check_circleDevelop and continually adapt care plans in collaboration with medical providers
Requirements
- verifiedGraduate of an accredited School of Nursing
- verifiedCurrently licensed as a Registered Nurse in the State of the posted location
- verified2+ years previous experience working in care management and/or with CKD/ESRD patients
- verifiedBasic Life Support (BLS) certification
