The Care Manager – Registered Nurse (Float) is a key member of an integrated Care Team, working alongside Advanced Practice Providers and Social Workers. This role exists to change the narrative in healthcare where patients often become secondary to processes; at Monogram, we put the patient and their quality of life at the forefront.
As a Care Manager RN, you will build trusting relationships with a small panel of patients who often struggle with serious diseases. Your day involves performing in-home care management visits, monitoring biometric data, and coordinating with medical providers to adapt care plans. You will serve as the primary point of contact for patients, offering education on kidney disease and treatment modalities, facilitating conversations about advance care plans, and supporting patients through vulnerable transitions like hospital discharges. This role allows you to directly experience the impact of your care as you help patients improve their quality of life and slow the progression of kidney disease.
Candidates selected for this role will be expected to obtain Basic Life Support (BLS) certification, which the company supports through onboarding. The position requires frequent local travel for in-home visits and may involve infrequent domestic travel to Brentwood, TN for training. Successful candidates must be self-starters capable of working independently and taking remote calls on nights and weekends as needed.
