About the Role
The Care Manager serves as a vital member of the interdisciplinary team, dedicated to assessing, planning, advocating for, and coordinating patient care from admission through discharge. The primary goal is to ensure a safe and effective transition post-hospitalization.
Operating within the Care Management model, this role provides effective transition planning and length-of-stay oversight. By utilizing performance metrics and adopting best practices, the Care Manager ensures the highest standards of patient experience, safety, and quality of care while remaining consistent with the Mercy mission, values, and service standards.
