About the Team
The Case Management department at Mercy Hospital Jefferson is comprised of dedicated nurse care managers and social workers. We touch the lives of patients in the emergency department and on inpatient medical floors. Our main goal is to discharge patients to the next most appropriate level of care, ensuring a safe transition post-hospitalization.
About the Role
As a Care Manager, you will be part of an interdisciplinary team that assesses, plans, advocates for, and coordinates care from admission to discharge. You will utilize performance metrics and best practices to provide effective transition planning and length of stay oversight, maintaining patient experience, safety, and quality of care.
Every day is different with a new challenge, so you will not become bored! You will formulate individualized discharge plans in collaboration with the patient, family, multidisciplinary team, and community resources. Requirements
- verifiedMaster's in Social Work or completed MSW requirements
- verified0-2 years acute care hospital experience
- verifiedCurrent Social Work license (LMSW, LCSW)
- verifiedBLS (CPR) certification at hire or within 90 days
- verifiedGraduate of accredited School of Nursing
- verified2-3 years acute care hospital experience preferred
- verifiedCurrent nursing license in state of employment
- verifiedCare Management or Utilization Management experience preferred
Nice to Have
Certification in Case Management. 2-3 years of experience in an acute care hospital setting. Care Management or Utilization Management experience.
