Key Responsibilities
- check_circleProvide supportive services including outreach, monitoring, assessment, and services planning
- check_circleCoordinate and monitor needed services for mental health and substance use disorder recovery
- check_circleUtilize de-escalation and crisis diversion skills to support participants in crisis
- check_circleComplete referrals and follow-up to all identified behavioral health supports
- check_circleEnsure coordination and collaboration with staff and community members
- check_circleBuild support networks and provide mentoring and coaching to participants
- check_circleMaintain productivity standards and complete all required documentation
Requirements
- verifiedBachelor's in human services field with relevant coursework
- verifiedValid driver's license
- verifiedDriving record in compliance with St. Luke's Penn Foundation policy
