As a Care Manager in North Haven, CT, you will play a vital role in supporting older adults as they navigate life's transitions. This position exists to ensure clients can live safely and independently by developing individualized care plans and coordinating essential services. You will work hand-in-hand with families, healthcare professionals, and community partners to deliver high-quality, person-centered support that truly makes a difference.
Your day will involve collaborating with interdisciplinary teams to ensure comprehensive, seamless care, advocating for clients, and connecting them to appropriate community-based resources. While the role includes field-based responsibilities, it offers a flexible hybrid workweek combining in-office and field duties Monday through Friday, with no weekend or evening hours required.
