The Behavioral Health Case Manager at Bell Hospital plays a pivotal role in ensuring safe, efficient, and compassionate care for patients of all ages and their families. This position is designed to bridge the gap between inpatient treatment and community reintegration. The primary mission is to collaborate closely with the behavioral health care team to facilitate the coordination of services during hospitalization and to execute a robust discharge plan that supports the patient's long-term well-being.
As a key member of the interdisciplinary team, you will serve as an advocate for patient choice, working within a patient-centered environment to protect health, safety, and rights. Your day-to-day involves initiating family conferences, conducting comprehensive psychosocial assessments, and connecting patients with vital community resources. From navigating complex insurance and state waiver qualifications to coordinating placements in group homes or sober living arrangements, you will ensure that every patient has a clear, realistic path forward upon discharge.
