This position is dedicated to supporting a defined population by collaborating closely with the healthcare team to coordinate appropriate, cost-effective services. The primary mission is to align with each member's plan of care, helping them achieve their optimal level of independence while enhancing their overall quality of life.
In this role, you will serve as a vital advocate and resource navigator. Your day-to-day involves conducting comprehensive psychosocial assessments to evaluate patient goals, social support systems, and functional limitations. You will work in close partnership with nurse case managers to develop and monitor care plans that promote patient functioning. A significant portion of the role focuses on identifying and facilitating access to organizational and community resources, ensuring they are implemented efficiently and in a timely manner.
You will also provide supportive counseling and education to members, families, caregivers, and the broader healthcare community, including guidance on end-of-life issues and Advanced Directives. The role requires maintaining high social work standards as defined by the NASW Code of Ethics and staying current with laws, regulations, and recent innovations in patient care. For inpatient settings, the role includes completing specific Uniform Assessment Instruments (UAIs) as required by Virginia Medicaid.
Note: This role is eligible for a $6,750 sign-on bonus.

Experience with computer software programs in a Windows environment. Knowledge of community systems and resources in the defined service area. Knowledge of regulatory issues for the Mid-Atlantic area. Previous case management or discharge planning experience.
