About the Role
This immediate opening is for a Licensed Clinical Social Worker (LCSW) based in Mission Viejo, CA. You will serve as a Short Term Acute Care Inpatient Medical Social Worker, focusing on Medical Surgical units. Your day will involve conducting initial evaluations and biopsychosocial assessments to develop comprehensive plans of care that meet patient and caregiver needs.
In this role, you will coordinate services for home care and equipment following discharge, referring patients to rehabilitation, long-term care facilities, and other necessary resources. You will participate in care conferences, facilitate discharge and contingency planning, and provide crisis intervention. A key part of your responsibilities includes educating patients and caregivers to facilitate successful discharge, while navigating complex systems such as Medicaid, Medicare, and the Department of Children and Families (DCF) process. You will also collaborate with internal and external team members to ensure quality care is delivered in a cost-effective manner.
