About the Role
We are seeking a clinically experienced Registered Nurse to join our Home Health team. This role is designed for a fast learner with exceptional communication skills who is motivated to help patients maintain their independence. As a visiting RN, you will travel to patient homes to administer services, including at-home IVs, dressing changes, and wound cleaning. You will conduct in-depth assessments, oversee treatment plans, and keep both doctors and the agency informed of patient health updates. Documentation and communication with the agency are managed through Synergy Software.
Availability & Schedule
Our patients require care seven days a week. While we offer scheduling flexibility, we ask that you are able to commit to at least two to three days each week (or ten productivity units), one weekend per month, and two holidays per year (one in winter and one in summer). Your work area will be a city close to your residence, serving a group of patients needing care for that specific day.

Key Responsibilities

- check_circlePerform re-certifications, transfers, and discharges as requested
- check_circleConduct initial and comprehensive patient assessments
- check_circleSupervise Home Health Aide visits per regulatory requirements
- check_circleDocument all patient interactions and upload data to the agency portal
- check_circleInitiate, re-evaluate, and revise patient plans of care
- check_circleAssess the need for additional team member services like PT or OT
- check_circleCoordinate care with clinical managers, physicians, and team members
Requirements
- verified1 year Visiting RN experience

- verifiedCurrent CA RN License
- verifiedBLS from American Heart Association or American Red Cross
- verifiedNSO Nursing License Insurance
- verifiedIV/Phlebotomy Certified
