The Jewish Board's Community Care Management programs provide compassionate, high-quality, evidence-based services to individuals and families in the communities we serve. Our staff utilize a culturally competent, person-centered approach to help individuals and their families develop skills and resources to improve overall functioning, instill hope, and strengthen resiliency. We work closely with community partners to address health disparities in our neighborhoods while celebrating the strengths and resilience of our communities.
Care Coordinators serve as vital links for adults and children with chronic behavioral health and medical conditions, connecting them to the services they need to stay as healthy as possible. This role is designed to inspire members to utilize these services to optimize their health outcomes. Working primarily in the field within a team setting, Care Coordinators assess risk and needs, develop person-centered care plans, and provide ongoing care management services.
In this evolving program, you will track and arrange appointments, educate members, and coordinate various aspects of their health and community services. The role involves integrating medical, specialized, and behavioral health services alongside social and educational support. You will partner with members, families, and other providers to create Individualized Care Plans and ensure access to essential services such as acute and preventative medical care, home health, housing, and state entitlements. As an integral part of the team, you will also share knowledge to support overall service provision and respond to member crises during and occasionally outside of regular business hours.
