About the Role
Sentara Health Plans is seeking a Full Time Care Coordinator to support patients in the Tidewater Road, VA area. This role performs a variety of casework duties and provides case management services to patients, families, and designated caregivers. The Care Coordinator is responsible for developing, participating in, and monitoring the multidisciplinary collaboration of services for patients where appropriate. A key aspect of the position involves assisting adult patients and their families with personal and environmental difficulties associated with medical conditions, extending through terminal illnesses.
Work Environment
This position requires travel for in-person, face-to-face assessments within the Tidewater Roads area. On days without scheduled assessments, employees may work remotely. Applicants must be located in the Tidewater area.
