About the Role
The Bilingual Care Navigator is a telehealth registered nurse dedicated to providing compassionate, clinically informed support to patients navigating rare or complex diseases. Acting as the primary point of contact, this role serves as a vital link between patients, healthcare provider offices, infusion sites, and internal and external teams. The position ensures timely therapy initiation by guiding patients through insurance processes, resolving access barriers, and exploring benefits and affordability options.
Focused on continuity of care, the Care Navigator offers individualized adherence coaching and maintains elevated levels of engagement throughout the entire treatment journey. By combining clinical expertise with empathetic patient training, the role empowers patients to manage their therapy effectively while ensuring efficient coordination of their care journey.

Requirements

Nice to Have
Clinical experience in infusion and/or rheumatology. Experience with adherence outreach, including familiarity with motivational interviewing or behavioral coaching. Previous experience engaging with multiple stakeholders such as healthcare offices, infusion centers of care, and specialty pharmacies. Ability to resolve complex issues with urgency, empathy, professionalism, and a positive outlook. Ability to adapt to a fast-paced, evolving healthcare environment and navigate multiple systems. Familiarity with adverse events and product complaint identification and reporting.
