About the Role

The Care Navigator is a telehealth registered nurse dedicated to providing compassionate, clinically informed support to patients navigating rare or complex diseases. Acting as the primary point of contact, this role serves as a vital link between patients, healthcare provider offices, infusion sites, and internal and external teams. The position ensures timely therapy initiation by guiding patients through insurance processes, resolving access barriers, and exploring benefits and affordability options.

Focused on continuity of care, the Care Navigator offers individualized adherence coaching and maintains elevated levels of engagement throughout the entire treatment journey. By combining clinical expertise with product knowledge and empathetic patient training, the role ensures efficient patient journey coordination and optimal health outcomes. The Care Navigator is expected to exercise independent judgment, maintain meticulous documentation, and foster strong professional relationships with patients, caregivers, and stakeholders to empower patient self-management.
