The Case Management Coordinator serves as a vital link for Medicaid Long Term Care and Comprehensive Program enrollees, focusing on telephonic and face-to-face assessments to evaluate medical needs and facilitate overall wellness. This role is designed to manage a caseload of supportive and medically complex members, developing proactive strategies to enhance both short and long-term health outcomes.

In this position, you will coordinate all case management activities, determining appropriate services such as prior authorizations, provider coordination, medication reviews, and community resource integration. The role requires utilizing critical thinking and negotiation skills to secure necessary healthcare options while employing motivational interviewing techniques to promote member engagement and lifestyle changes. You will empower members to actively participate in their healthcare decision-making, ensuring they make informed, independent choices regarding their medical and healthy lifestyle paths.
