The Admissions Coordinator serves as the vital first point of contact for individuals and families seeking hospice services. This role is designed to bridge the gap between referral sources and our clinical teams, ensuring a smooth, empathetic, and efficient admission process. You will be the face of Catholic Hospice, guiding patients and families through their initial inquiries and coordinating the scheduling of admission evaluations.
In this position, you will manage a dynamic workflow that requires balancing multiple priorities. Your day will involve taking telephone inquiries, gathering essential demographic and medical information, and verifying terminal diagnoses with referring physicians. You will collaborate closely with Marketing, field admissions, and the Interdisciplinary Teams to ensure that every referral is handled with care and urgency. A key part of your responsibility is to educate families about hospice services and reimbursement options, helping them make informed decisions during a difficult time.
This role demands a high level of confidentiality and sensitivity, as you will be handling private health information and navigating complex family dynamics. You will also play a crucial role in expanding our referral base by maintaining strong relationships with doctors, social workers, and case managers. Whether you are coordinating overnight mail for out-of-state families, ensuring pharmacy compliance, or supporting the team during evenings and holidays, your work directly impacts the speed and quality of care our patients receive.
