About the Role

The Admissions Coordinator serves as the vital first point of contact for individuals and families seeking hospice services. This role is designed to bridge the gap between referral sources and our clinical teams, ensuring a seamless and empathetic transition into care. You will be the face of Catholic Hospice, guiding patients and families through the initial inquiry process with clarity and compassion.

In this position, you will coordinate the scheduling of admission evaluations, manage a high volume of telephone inquiries, and facilitate communication across Marketing, field admissions, and Interdisciplinary Teams. Your day-to-day involves verifying patient information, obtaining necessary consents, and collaborating with finance and pharmacy departments to ensure quick and accurate admissions. You will also play a key educational role, helping staff and families understand hospice reimbursement and services.

This is a dynamic role requiring the ability to manage competing demands and tight timelines while maintaining strict confidentiality. As a member of the admission team, you will participate in a 7-day coverage schedule, including evenings and holidays, ensuring our services are always available to those in need.
