About the Role

The Admissions Coordinator serves as the vital first point of contact for individuals seeking hospice services. This role is designed to bridge the gap between potential patients, their families, and our interdisciplinary care teams. You will be responsible for managing telephone inquiries, coordinating the scheduling of admission evaluations, and ensuring a seamless transition into care.

In this position, you will act as a liaison between Marketing, field admissions, and referral sources, including physicians, social workers, and case managers. Your day will involve gathering essential patient information, verifying terminal diagnoses and prognoses, and obtaining necessary consents. You will work closely with the Admission RN to facilitate appointments and ensure that all financial and reimbursement avenues are explored prior to admission.

Beyond administrative coordination, you will play a key educational role by explaining hospice services to patients and families within 24 hours of contact. You will also support the broader team by maintaining accurate documentation, monitoring status logs, and assisting with staff education regarding reimbursement issues. This role requires the ability to handle sensitive information with strict confidentiality while managing competing demands and tight timelines in a fast-paced healthcare environment.
