This position provides spiritual support and counseling to patients, families, and staff across home and in-patient settings. You will serve as a vital liaison between patients, families, and their local congregations to identify and meet spiritual needs throughout the hospice journey. The role involves coordinating bereavement activities, assisting with funeral and memorial services, and actively participating in the Hospice Interdisciplinary Team to resolve care problems.
In this capacity, you will assess spiritual and bereavement care needs, document interventions, and collaborate with the Hospice Bereavement & Volunteer Manager to lead community support groups. You will also contribute to community outreach, including the annual Hospice Interfaith Memorial Service and holiday programs, while serving as an instructor for volunteer training sessions.
Candidates must possess a valid driver's license and current auto liability insurance to fulfill the essential functions of this role.
