About the Role

This Physical Therapist position is ortho-focused and operates within a mobile outpatient model, primarily serving patients in Assisted Living Facilities (ALF), Independent Living Facilities (ILF), and community settings in Vernon, CT. Powerback delivers an empowered approach to recovery by providing integrated and individualized solutions that transform the road to recovery for everyone.

The role centers on patient-centered care that provides a sustainable way of feeling better, moving better, breathing better, and living better. You will primarily provide rehabilitation services in an in-home setting, whether that be a patient's private residence or their room in a facility. This "Powerback Rehabilitation To You" model allows you to evaluate and treat patients in the most functional environment possible. You will assess needs, develop and deliver physical therapy programs, and instruct patients and caregivers in skills essential for promoting independence. The position offers a flexible schedule and work environment designed to help patients get their power back.
