Travel Physical Therapist (PT)

About the Position

Pride Health is seeking a dedicated Home Health Physical Therapist to support a healthcare facility in Virginia Beach, VA (23464). This role is designed to deliver essential care directly within patients' homes, ensuring continuity of treatment and safety in their personal environments.

The Role

As a Home Health Physical Therapist, you will conduct comprehensive evaluations to assess mobility, strength, balance, and functional limitations. Your day will involve developing individualized treatment plans with measurable goals, providing therapeutic exercises, and training patients and caregivers on home exercise programs and fall prevention. You will also assess home environments for safety risks, coordinate care with physicians and interdisciplinary teams, and manage critical documentation such as OASIS forms.

This is a contract position with a schedule of Monday through Friday, 8:30 AM to 5:00 PM. The initial contract length is three months, with the possibility of extension or conversion. You will be responsible for traveling daily to patient homes within a 50-mile radius of Virginia Beach, Norfolk, Portsmouth, and surrounding flexible areas. Mileage reimbursement is provided for travel.

Hiring Process
