About the Role
The Weekend Clinical Care Coordinator serves as the critical clinical liaison for Home Health and Hospice services in Dedham during weekends. Reporting to the Sr. Director of Home and Community-Based Services, this role ensures continuity of care when standard weekday operations are reduced. The position exists to manage the flow of patient referrals, coordinate urgent clinical responses, and maintain seamless communication between patients, families, and field clinicians.
In this hybrid capacity, you will act as the primary decision-maker for weekend clinical inquiries. Your day involves monitoring incoming referrals through systems like Allscripts and 4 Next, verifying managed care coverage, and utilizing professional nursing judgment to determine necessary patient interventions. You will coordinate admission visits, adjust weekend schedules in real-time to accommodate staffing or patient needs, and provide guidance to home care providers regarding clinical issues or incidents.
A key component of this role is the handoff process. You will receive critical updates from the Clinical Manager regarding potential patient or staff issues and will compile a comprehensive weekend summary report for the Intake Department and leadership to review on Monday mornings. This ensures that the transition from weekend to weekday operations is smooth and that no patient needs are overlooked.
Hiring Process
Candidates selected for this position will be expected to hold a current Massachusetts license as a Licensed Practical Nurse (LPN) with at least two years of experience in certified home care or hospice. The role requires proficiency in EMR documentation (specifically EPIC) and a working knowledge of Medicare regulations.

Requirements
