About the Role

As a Wellness Nurse at Ashby Ponds, you will be the guardian of our residents' health and wellness. This role exists to ensure the overall well-being of our community members by monitoring their health status and advocating for their needs. You will serve as a consistent caregiver, building meaningful relationships with residents and their families while supporting their physical, spiritual, emotional, and clinical needs.

Your day will involve observing and evaluating residents to identify changes in clinical condition, updating service plans as necessary, and ensuring all medical needs are met. You will play a critical role in coordinating ancillary support services, such as home health, hospice, and outpatient rehab, while collaborating closely with Assisted Living Managers, Memory Care Managers, and the Wellness Manager to analyze care outcomes. Additionally, you will assist in training and monitoring care associates regarding medication care, ensuring that every medication and treatment is administered accurately and documented correctly.
