The Denver Hospice is seeking a Weekend RN to provide skilled nursing coordination of care to patients ranging from neonates to geriatrics. As a member of the Interdisciplinary Team, you will collaborate across disciplines to coordinate patient care and participate in Plan of Care meetings. Your role involves adapting care to meet the needs of patients at their place of residence, including homes, long-term care, or assisted living facilities.
You will demonstrate appropriate nursing care by communicating with attending providers regarding medications and supplies, while supervising Certified Nurse Aides and delegating specific tasks per agency policy. A key part of this position is educating patients, families, caregivers, and the community regarding hospice philosophy and palliative treatments. You will also develop and maintain professional business relationships with facilities by providing excellent customer service to both patients and facility staff.
